How Does IVF Fertility Treatment Work?


In vitro fertilization (IVF) is one of the most common and effective assisted reproductive technologies (ART) used to help individuals or couples conceive a child. It involves a multi-step process of fertilizing an egg outside the body and then transferring the resulting embryo into the uterus. IVF has provided hope for many dealing with infertility due to various medical or unexplained reasons. Let’s explore how IVF works and the specific stages involved.

Steps in the IVF Process

1. Ovarian Stimulation
The IVF process begins with ovarian stimulation, where hormone injections (usually follicle-stimulating hormone or FSH) are administered to the woman to promote the development of multiple eggs. Typically, a woman produces one egg during her natural cycle, but in IVF, doctors aim to retrieve multiple eggs to increase the chances of successful fertilization.

Throughout this stage, the progress is monitored through ultrasound scans and blood tests to check hormone levels and the growth of the ovarian follicles, which contain the eggs.


2. Egg Retrieval
Once the eggs have matured (usually after 8-14 days of hormone stimulation), the next step is egg retrieval. Under mild anesthesia, a needle guided by ultrasound is used to extract the eggs from the ovarian follicles. This is a relatively quick procedure, often lasting about 20 minutes. The eggs are then taken to a laboratory for the fertilization process.


3. Sperm Collection and Preparation
At the same time as egg retrieval, a sperm sample is collected from the male partner or a sperm donor. The sperm is washed and concentrated to select the healthiest and most motile sperm. In cases of severe male infertility, intracytoplasmic sperm injection (ICSI) may be used, where a single sperm is directly injected into an egg to facilitate fertilization.


4. Fertilization
The eggs and sperm are combined in a laboratory dish for fertilization. In conventional IVF, many sperm are placed around each egg, allowing one to penetrate the egg naturally. If successful, fertilization occurs within hours, and the fertilized egg becomes a zygote. Within 3-5 days, the zygote will develop into a blastocyst (an embryo with multiple cells).


5. Embryo Culture
After fertilization, the embryos are cultured in a lab under optimal conditions for several days. During this time, embryologists monitor the development of each embryo to assess its health and viability. In some cases, genetic testing may be done to screen for chromosomal abnormalities before embryo transfer.


6. Embryo Transfer
When the embryos are ready, typically after 3-5 days, the best-quality embryo (or embryos) is selected for transfer into the uterus. Using a thin catheter, the embryo is placed into the uterus through the cervix. The procedure is relatively simple and painless, often compared to a pap smear.

To minimize the risk of multiple pregnancies, many clinics now transfer a single embryo, known as single embryo transfer (SET). However, depending on the patient’s circumstances, two or more embryos may be transferred.


7. Luteal Phase Support and Pregnancy Test
After the embryo transfer, the patient may receive progesterone supplements to support the uterine lining and facilitate implantation. A blood test is usually performed about 10-14 days later to confirm pregnancy.



Success Rates and Considerations

The success rate of IVF depends on various factors, including age, the underlying cause of infertility, and the quality of the eggs and sperm. According to the American Society for Reproductive Medicine (ASRM), for women under 35, the live birth rate per IVF cycle is about 50%, but the success rate decreases significantly with age, particularly after 40.

It is important to note that IVF can be both emotionally and physically demanding. The process may require multiple cycles for success, and there are risks such as ovarian hyperstimulation syndrome (OHSS) and the chance of multiple pregnancies. Additionally, the high cost of IVF is a concern for many patients, as the procedure can cost thousands of dollars per cycle.

Recent Advances in IVF

IVF has seen remarkable advances in recent years. Techniques like preimplantation genetic testing (PGT) allow doctors to test embryos for genetic abnormalities before transfer, improving success rates and reducing the likelihood of miscarriage. Additionally, the freezing of eggs and embryos through vitrification has become more efficient, enabling couples to preserve embryos for future use.

Artificial intelligence (AI) is also being integrated into the IVF process, helping embryologists to better select viable embryos based on subtle factors that might be difficult to identify with the naked eye.
